    I hereby authorize Rocky Mountain Summer Conservatory to publish the photographs taken of me (or my minor child), and my (or the minor child’s) name, for use in printed publications and website. I acknowledge that I (nor the minor child) will not receive compensation for the use of photos. I further agree that said photos and names in any publication and website produced by RMSC confers upon me (or the minor child) no rights of ownership whatsoever. I release RMSC, its vendors and its employees from liability for any claims by me or any third party in connection with my participation.

  • Leave of Campus for Minor Child

    This is to certify that my minor child has permission to leave the campus of Rocky Mountain Summer Conservatory (under the supervision of the Student Life Staff) during the period in which (s)he is enrolled as a student, if the activity of leave is considered safe by RMSC. I also agree that RMSC will not be responsible for my child while (s)he is away from the supervision of RMSC.

    I, the undersigned, hereby give my full consent and permission for my minor child to attend Rocky Mountain Summer Conservatory in Steamboat Springs, Colorado.
