Before You Go Student Forms

After acceptance, please submit by JUNE 1 the following Student Forms.

1. Student Code of Conduct
2. Travel Form
3. Release and Permission Form
4. Activities Release Form
5. Student Life Request Form

At 24-48 hours prior to your Arrival:

1. Submit negative test results to the COVID-19 Test Submission Form.

Bring to Check-In:

1. Medical Release and Physical Examination Form – A downloadable form. Please include your insurance information.
